Frequently Asked Questions

1:1 Medical Nutrition Therapy

  • $140 for the Initial Consultation which is 60 minutes
  • $120 for 60 minute follow-up
  • $90 for 45 minute follow-up
  • $60 for 30 minute follow-up


Intuitive Eating Group $320

  • option to pay in full at time of registration
  • option to pay $40 at the time of each session - registration requires commitment to pay for all 8 sessions, payment will be auto-charged on the day of each session


Professional Fees: $30 per 15 minutes

Charges will be incurred for work done on behalf of clients outside of sessions that lasts longer than 10 minutes and cannot be billed to insurance. This may include collaboration of care with other providers, phone and messaging that lasts longer than 10 minutes, attendance at meetings on client's behalf, and time spent performing any other related services requested by a client.


Yes, we can bill insurance for 1:1 services for medical nutrition therapy. We are in-network with Anthem Blue Cross Blue Shield, United Health Care/UMR, and Medicare. 


No, we cannot bill Medicaid, Blue Cross Blue Shield of Michigan, or Healthy Indiana Plans at this time.


No, we cannot bill insurance for group classes or coaching services


All payments are processed through IntakeQ, the client portal, using a credit card. Options to pay with HSA and FSA are available. We cannot accept checks or cash as payment. 


A credit card reserves the appointment, however will not be charged at the time of booking. You can expect a charge 

  • on the day of the appointment for client's who select to self-pay 
  • if insurance does not cover a session, there is a deductible to meet, or there is a co-pay/co-insurance associated with rendered services
  • if there is a no show to an appointment or a late cancellation 


Under the No Surprises Act (H.R. 133 - which went into effect on January 1, 2022), health care providers need to give clients or patients who do not have insurance or who are not using insurance an estimate of the bill for medical items and services.


Self-pay clients can expect to receive a GFE within the client portal before or during the first session.


At this time all sessions are offered virtually. Sessions are hosted via Google Meets. 


The first session is a getting-to-know-you process. We will reasons for seeking consultation, mental heath, medical, and surgical history, and current eating patterns. You also get to ask questions and get to know your provider as well. During the initial session we will discuss the types of nutrition plans of care that may be available to meet your needs, however most clients will not receive specific nutrition education or plans of care until the second session. Our goal is to gather all of the important information about you necessary to create a plan of care individualized for you.


We are a group of weight-inclusive providers who believe that health can be achieved while living in a wide variety of shapes and sizes. To that end we do not offer services for intentional weight loss or provide calorie counts or macronutrient distribution ranges.


Active clients can request a copy of session notes by sending a written request through the client portal. In order to protect your privacy and maintain compliance with federal regulations for Heath Information Privacy active clients must complete a Release of Information form for any person or third party with whom you wish to collaborate in care and/or receive specific medical records.


Former clients who are no longer actively receiving care need to complete a Release of Records form  and fax to 574-440-8701. Allow a two (2) weeks for processing.


Please note a copying charge of $0.25 per page will be incurred for printed or faxed medical records. Medical records cannot be modified in order to reduce the page count.
